Maternal Child Health Nurse - Nursing

Responsibilities

• Accesses and evaluates the health care needs of patients and families
• Develops a care plan based on patient needs, physician orders, nursing assessment parameters and identified goals
• Implements the care plan and revises it as necessary
• Administers medications and treatments as prescribed by the physician
• Teaches the patient and family/caregivers self-care techniques
• Provides case coordination, health teaching, supportive counseling and advocacy
• Completes, maintains and submits all required documentation
• Performs complete prenatal, postpartum and infant assessments using EHR documentation
• Performs other duties as assigned

Requirements

• Physical requirements include standing and sitting for extended periods
• Work environment includes ambulatory clinics, inpatient-hospital visits, home visits, community events and offices
• May require working with challenging patients
