NP/PA - House Calls Travel Provider

About the role

Join a patient-centered, field-based role as a HouseCalls Advanced Practice Clinician (Nurse Practitioner or Physician Assistant).

Responsibilities

• Conduct comprehensive in-home health assessments, including medical history, medication reconciliation, vital signs, and screenings.
• Identify diagnoses for care management and collaborate with primary care providers.
• Recognize urgent situations and provide education on diseases, medications, and health management.
• Address social determinants of health and make appropriate referrals.

Requirements

• Active, unrestricted licensure and relevant certification (ANCC, AANP, or NCCPA).
• Valid driver’s license and reliable transportation.
• Ability to navigate home environments and perform assessments requiring sitting, standing, or kneeling.

Qualifications

Preferred: 1+ years of clinical experience, proficiency with electronic records, and strong communication skills with diverse populations.

Schedule

Role offers flexible scheduling, with no on-call, weekends, or holidays.

Travel

Travel across North and South Carolina is required, with expenses covered.

Pay

Compensation ranges from $109,500 to $164,000 plus incentives and benefits.
