Mental Health Case Manager

Responsibilities

  • Act as a liaison between therapist, the referring agency, and family of the client.
  • Arrange family visits, coordinate transportation, and ensure clients’ needs are met.
  • Ensure communication between family, referral source and Mahajan therapeutics is agreeable and ongoing.
  • Attend court hearings as deemed appropriate.
  • Ensure documentation occurs according to policy.
  • Attend treatment staffing with prepared report that accurately represents client’s response to treatment.
  • Facilitate individual and group CSPT as assigned.
  • Function on agency quality assurance or other committees as assigned.
  • Assist with in-service training as directed by Clinical Director or CM Supervisor.
  • Attend supervisory sessions with prepared agenda.
  • Ongoing assessment of client needs
  • Assistance in achieving personal independence in managing basis needs as identified by the client and/or parent or guardian
  • Facilitation of further development of daily living skills, if identified by the client and/or parent or guardian
  • Coordination of the ISP, including: Services identified in the ISP
  • Assistance with accessing natural support systems in the community
  • Linkages to formal community services and systems
  • System monitoring
  • Crisis management and stabilization
  • Advocacy and outreach
  • Activities that increase the client’s capacity to positively impact the client’s environment
  • Adequate knowledge and ability to navigate EHR (Electronic Health Record) system.
  • Assume additional duties as assigned by Clinical Director or CM Supervisor.

Qualifications

  • 1 year experience working with adolescents, teens, and adults, emotionally disturbed population, mental health experience
  • In a pleasant and professional manner with other employees, physicians, providers, patients, and patient’s families to promote a team effort through cooperation and respect for fellow employees.
  • Proven competency with MS Office applications, including Word, Excel and PowerPoint required.
  • Adaptability in a challenging and dynamic environment; understand and work well in a matrixed organizational structure required.
  • Satisfactory references from employers and/or professional peers and satisfactory criminal background check.
  • Satisfactory drug screen
