Medical Care Manager

About the role

A Medical Care Manager contributes to AHF by utilizing their experience, skills, and education to thoroughly understand the Ryan White Care Act Program. You will work with providers and the Healthcare Center team to ensure every Ryan White client receives the support they require and access to available resources, including education and assistance for program eligibility.

Working directly within the Healthcare Center, you will meet with clients to complete Needs Assessments, develop Action/Care Plans, and provide ongoing education about diagnoses and medications. You will reengage clients who dropped out of care or support new patients at high risk of dropping out based on risk assessments, missed appointments, or service utilization data.

For Ryan White contracts that include psychosocial needs, you will address mental health and basic needs such as housing, food, and clothing by developing relationships with relevant programs and agencies to facilitate referrals and support. You will conduct ongoing retention calls to ensure continuity of care and uphold collaboration across divisions.

A Medical Care Manager enhances AHF’s reputation by meeting clients where they are, identifying strengths and opportunities for growth, and assisting with realistic goal setting. You will coordinate care for people living with HIV (PLWH) to ensure they achieve and maintain viral load suppression and retain medical care, embodying the safety net role of the Ryan White Care Act for uninsured HIV/AIDS patients.

Responsibilities

• Complete Needs Assessments and develop Action/Care Plans for clients.
• Provide ongoing education to patients about their diagnoses and medications.
• Reengage clients who have dropped out of care or support high-risk patients.
• Address psychosocial needs, including mental health, housing, food, and clothing, by facilitating referrals to community programs and agencies.
• Conduct ongoing retention calls to ensure continuity of care and radical cooperation across divisions.
• Coordinate care for people living with HIV (PLWH) to achieve and maintain viral load suppression and retention in medical care.
• Verify client eligibility for the ACA (Affordable Care Act) program and enroll those who qualify.
• Collaborate with providers to develop and monitor individual service plans, reviewing care plans at least every six months.
• Ensure timely and coordinated access to medically appropriate health and support services.
• Advocate for AHF’s top initiatives while upholding the organization’s Core Values and Mission.

Requirements

• Bachelor’s or Master’s degree in Social Science, Social Work, or Human Services.
• Minimum of 3 years of Case Management experience (preferred).
• Knowledge of Ryan White Services (preferable).
• Knowledge of community resources.
• Competence in cultural and linguistic diversity.
