Medical Care Manager

About the Role

A Medical Care Manager contributes to AHF by utilizing their experience, skills, and education together with a thorough understanding of the Ryan White Care Act Program. You will work with providers and the healthcare center team to ensure every Ryan White client receives the support they require and access to available resources, including education and assistance to understand program eligibility.

Working directly within the Healthcare Center, you will meet with clients to complete Needs Assessments, develop Action/Care Plans, and provide ongoing education about diagnoses and medications. For contracts combining psychosocial needs with Medical Care Management, you will address mental health needs and basic needs such as housing, food, and clothing by developing relationships with relevant programs and agencies to facilitate referrals and support.

As a Medical Care Manager, you will enhance AHF’s reputation by meeting clients where they are, identifying strengths and opportunities for growth, and assisting with realistic goal setting. Your collaboration and engagement will bring the essence of the Ryan White Care Act—serving as a safety net for uninsured HIV/AIDS patients—to our most vulnerable population. Your work will directly contribute to AHF’s success in meeting Ryan White contract deliverables and measures established by the HIV/AIDS Bureau (HAB) and the Department of Health and Human Services.

You will advocate passionately for AHF’s top initiatives while ensuring the organization’s Core Values and Mission remain central to your work. AHF operates in a dynamic, collaborative environment where staff are accountable to multiple leaders. As part of a close-knit team, you will act as a liaison between providers and clients, facilitating the transition from clinical visits to implementing optimal care plans.

Through brief, goal-focused interventions, you and the client will work together toward viral suppression, improved health outcomes, and enhanced quality of life.

Requirements

• Bachelor’s or master’s degree in Social Science, Social Work, or Human Services.
• Minimum of 1 year of Case Management experience (preferred).
• Knowledge of Ryan White Services (preferable).
