Responsibilities

• Provide direct patient care, including personal hygiene, transfers, ambulation, and basic nursing tasks as instructed by healthcare professionals.
• Assist with meal preparation, light housekeeping, and engage clients in activities to promote well-being.
• Maintain accurate records, report changes in patient conditions, and follow the care plan strictly.
• Use equipment such as blood pressure cuffs and stethoscopes, and complete documentation via EVV app.
• Uphold patient confidentiality, privacy, and rights, and report any concerns like abuse or neglect immediately.
• Support client independence and adapt to individual family and patient needs.

Qualifications

• Current certification with DFS.
• High school diploma or GED preferred.
• Effective communication skills, ability to perform physical tasks, and a compassionate attitude towards patients.

Travel within the designated area is required. The role demands dependability, physical capability, and adherence to the established care plan.
