Role Overview: The Licensed Practical Nurse (LPN) provides direct, high-quality nursing care to acutely and chronically ill patients in their homes, under supervision. Key Responsibilities Deliver nursing services within legal scope and plan of care, ensuring infection control procedures.Accurately document patient visits and care activities in electronic records.Participate in care coordination by communicating patient needs, updates, and changes in condition with team members, patients, and families.Adapt to diverse home environments, ensuring respectful and competent care. Qualifications & Skills Current, unrestricted LPN license in the practice state.Valid driver’s license, vehicle insurance, or reliable transportation.Preferred: CPR certification, flexibility, independent work skills, strong communication, organization, and multitasking abilities.
