About the Role

The Home and Community Based Services Licensed Practical Nurse (LPN) provides direct patient care in patients' homes, documents clinical notes, and coordinates care activities to ensure high-quality services.

Responsibilities

• Deliver clinical services within the scope of practice, adhering to state laws and care plans
• Accurately document patient visits and progress in electronic medical records
• Participate in care coordination, communicate effectively with patients, families, physicians, and team members

Requirements

• Unrestricted LPN licensure in practice state
• Current CPR certification
• Valid driver’s license and reliable transportation or access to public transit
• Ability to work independently and handle diverse home situations

• Ability to manage multiple tasks effectively
