About the Role

The Home and Community Based Services Licensed Practical Nurse (LPN) provides direct nursing care to acutely and chronically ill patients in their homes, following physician prescriptions.

Responsibilities

• Deliver high-quality clinical services within legal scope, adhering to care plans and infection control protocols.
• Thoroughly document patient visits in electronic medical records and visit verification systems.
• Participate in care coordination by reporting patient needs, attending case conferences, and communicating with patients, families, and healthcare team members.
• Function effectively in diverse home environments, respecting all clients regardless of background.

Requirements

• Current, unrestricted LPN licensure in the practice state.
• Valid driver’s license, vehicle insurance, or reliable transportation.

Preferred Qualifications

• CPR certification.
• Ability to work flexible hours.
• Strong organizational and communication skills.
• Ability to work independently.
