About the Role

Provide direct nursing care to acutely or chronically ill patients in their homes. Work under the supervision of a Registered Nurse or supervisor, coordinating patient care activities to ensure high-quality services while adhering to all regulatory and safety protocols. The role emphasizes equitable, patient-centered care in diverse home settings.

Responsibilities

• Deliver direct nursing care to patients in their homes
• Document clinical visits and patient progress
• Coordinate patient care activities with healthcare team members
• Report patient needs and condition changes promptly
• Participate in care coordination and case conferences

Requirements

• Current unrestricted LPN licensure
• CPR certification or ability to obtain within 90 days
• Valid driver’s license and reliable transportation
• Ability to work effectively in diverse home settings

Pay

$21.70 – $32.55 per hour
