Role Overview
The Home and Community Based Services Licensed Practical Nurse (LPN) provides direct nursing care to acutely or chronically ill patients in their homes, under supervision of a Registered Nurse or supervisor.
Key Responsibilities
- Deliver high-quality clinical services within legal scope
- Document patient visits accurately in electronic records
- Participate actively in care coordination by communicating patient needs and changes in condition to the healthcare team
Qualifications
- Active, unrestricted LPN license in the state
- Current CPR certification
- Valid driver’s license with vehicle insurance
- Ability to work in diverse home environments
- For Louisiana: at least 1 year of LPN experience required
