About the Role
The Licensed Practical Nurse (LPN) provides direct patient care in home settings, following physician prescriptions and within the scope of practice defined by state laws. They collaborate with the healthcare team, document patient visits thoroughly, and participate in care coordination to ensure high-quality patient outcomes. The role requires flexibility to work in diverse home environments and effective communication with patients, families, and medical staff.
Responsibilities
- Deliver clinical services per the care plan, adhering to infection control protocols
- Document patient visits accurately in electronic medical records
- Report changes in patient condition and participate in case conferences
- Coordinate care activities to optimize patient well-being
Requirements
- Valid and unrestricted LPN licensure in the practice state
- Current driver's license, vehicle insurance, or access to transportation
- Minimum of 1+ years of LPN experience (specific to Louisiana)
Skills
- Preferred CPR certification
- Strong communication, organizational, and multitasking skills
- Ability to work independently and adapt to various home situations

Pay
