About the Role

The Licensed Practical Nurse (LPN) provides direct patient care in home settings, following physician prescriptions and collaborating with the healthcare team.

Responsibilities

• Deliver high-quality clinical services within legal practice scope, adhering to the care plan and infection control protocols.
• Document patient visits thoroughly in electronic medical records and visit verification systems.
• Participate in care coordination by reporting patient needs, attending case conferences, and communicating effectively with patients, families, and healthcare professionals.
• Adapt to diverse home environments and patient backgrounds, ensuring respectful and effective care.

Requirements

• Current and unrestricted LPN licensure in the practice state.
• Valid driver’s license, vehicle insurance, or access to public transportation.

Skills

• Preferred: CPR certification.
