About the role

Provide direct nursing care to patients in their homes, delivering high-quality clinical services within your scope of practice.

Responsibilities

• Deliver clinical services following the physician’s care plan and infection control procedures.
• Document patient visits thoroughly in electronic medical records and electronic visit verification systems.
• Participate in care coordination by reporting patient needs, attending case conferences, and communicating effectively with patients, families, physicians, and team members.

Requirements

• Current, unrestricted LPN licensure in the practice state.
• Current CPR certification.
• Valid driver’s license, vehicle insurance, or access to transportation.
• Ability to work independently in diverse home environments.
• Minimum one year of LPN experience (preferred for some regions).

Skills

• Home care experience (preferred).
• Flexibility, multitasking, and strong communication and organizational skills.

Pay

Hourly pay ranges from $22.90 to $34.35, based on experience and market factors.
