Role Overview

The Licensed Practical Nurse (LPN) provides direct patient care in home settings, following physician orders and within the scope of practice defined by state laws.

Responsibilities

• Deliver high-quality clinical services
• Document patient visits thoroughly in electronic records
• Coordinate care activities with the healthcare team
• Report patient needs and condition changes promptly
• Participate in case conferences
• Communicate effectively with patients, families, and providers
• Adapt to diverse home environments
• Ensure infection control procedures are followed

Qualifications & Skills

• Current, unrestricted LPN licensure in the practice state
• Valid driver's license, vehicle insurance, or access to reliable transportation
• CPR certification (preferred)
• Ability to work flexibly
• Strong organizational and communication skills
• Capacity to manage multiple tasks independently
