LPN - Homecare PDN PEDS

About the role

Provide in-home direct patient care, monitor patient conditions, administer medications, and support care plans for pediatric and adult patients. Focus on delivering personalized, stable, and structured care with less stress and more control.

Responsibilities

• Perform patient assessments and monitor health status
• Administer medications following care plans
• Support patients and families in home settings

Qualifications

• Active LPN license
• Current BLS certification
• Preferred 1+ year experience
