About the role
The Home and Community Based Services Licensed Practical Nurse (LPN) provides direct patient care in clients' homes, following physician prescriptions. They document clinical notes, coordinate care activities, and ensure high-quality services within legal scope and infection control standards. Reporting to a Registered Nurse or supervisor, they participate in care coordination, communicate effectively with patients, families, and healthcare team members, and adapt to diverse home environments.
Responsibilities
- Provide clinical services aligned with the plan of care
- Thoroughly document patient visits in electronic records
- Participate in care coordination and report patient needs and changes
Requirements
- Valid LPN license in the state of practice
- Current CPR certification
- Valid driver’s license, reliable transportation, or access to public transit
- Ability to work effectively in diverse home settings
