About the Role
As a Licensed Practical Nurse (LPN) in home and community-based services, you will provide direct nursing care to acutely and chronically ill patients in their homes.
Responsibilities
- Deliver high-quality clinical services within legal scope, following care plans and infection control protocols.
- Accurately document patient visits and care activities in electronic medical records.
- Participate in care coordination, report patient needs and changes, and communicate effectively with patients, families, and healthcare team members.
Requirements
- Current, unrestricted LPN licensure in the state of practice.
- CPR certification, valid driver’s license, and reliable transportation.
- Ability to work independently in diverse home environments and manage multiple tasks.
Preferred Qualifications
- Experience in home care.
- Strong communication, organizational, and writing skills.
