About the role
Provides direct nursing care to acutely and chronically ill patients in their homes, under the supervision of a Registered Nurse.
Responsibilities
- Administer clinical services within legal scope
- Document patient visits accurately in electronic records
- Participate in care coordination
- Communicate effectively with patients, families, and healthcare team
- Adapt to diverse home environments regardless of patient background
Requirements
- Current LPN licensure
- Valid driver’s license and reliable transportation
Skills
- CPR certification (preferred)
- Ability to work independently
- Manage multiple tasks
- Excellent communication skills
