About the Role

The Home and Community Based Services Licensed Practical Nurse provides direct nursing care to acutely or chronically ill patients in their homes, under the supervision of a Registered Nurse or supervisor.

Responsibilities

• Deliver high-quality clinical services within legal scope and plan of care, adhering to infection control protocols.
• Thoroughly document patient visits in electronic medical records and visit verification systems.
• Participate in care coordination by reporting patient needs, attending case conferences, and communicating effectively with patients, families, and healthcare team members.

Requirements

• Current, unrestricted LPN licensure in the practice state.
• CPR certification, valid driver’s license, and dependable transportation.
• Ability to work independently and adapt to diverse home environments.

• Experience in home care.
• Strong communication and organizational skills.
• Flexibility.

Pay

Ranges from $21.70 to $32.55 per hour.
