LPN Pediatric Home Care

About the role

Provide direct nursing care to acutely and chronically ill patients in their homes, following a physician’s plan of care. Coordinate patient care activities to ensure high-quality service, participate in care coordination, and communicate effectively with patients, families, and healthcare team members. Adhere to infection control procedures and document visits accurately in electronic medical records.

Responsibilities

• Provide direct nursing care to patients in their homes according to the physician’s plan of care.
• Record thorough clinical notes and coordinate patient care activities.
• Participate in care coordination and report changes in patient condition.
• Communicate effectively with patients, families, and healthcare team members.
• Adhere to infection control procedures.
• Document visits accurately in electronic medical records.

Requirements

• Current and unrestricted LPN licensure.
• CPR certification.
• Valid driver’s license, reliable transportation, or access to public transit.
• Ability to work independently in diverse home environments.
