About the Role

The Home and Community Based Services Licensed Practical Nurse provides direct patient care in clients' homes, following physician prescriptions and collaborating with the healthcare team.

Responsibilities

• Deliver high-quality clinical services within legal scope, adhering to the plan of care and infection control protocols.
• Accurately document patient visits and clinical notes in electronic medical records and visit verification systems.
• Participate in care coordination by reporting patient needs, attending case conferences, and communicating effectively with patients, families, and healthcare providers.

Requirements

• Valid and unrestricted LPN license in the practice state.
• Current CPR certification.
• Reliable transportation or access to public transit, and a valid driver’s license with insurance.
• Ability to work in diverse home environments and manage multiple tasks independently.

Preferred Qualifications

• Home care experience.
• Strong communication and organizational skills.
• Flexibility to meet client needs.
