About the Role
The Home and Community Based Services Licensed Practical Nurse provides direct nursing care in patients' homes, following physician prescriptions.
Responsibilities
- Deliver high-quality clinical services within legal scope and plan of care, collaborating with the healthcare team
- Document patient visits accurately in electronic medical records and visit verification systems
- Participate in care coordination by reporting patient needs, attending case conferences, and communicating with patients, families, and providers
- Adhere to safety and infection control protocols
Requirements
- Current unrestricted LPN licensure and CPR certification
- Valid driver’s license, dependable transportation or access to public transit
- Ability to work independently and adapt to various home environments
Preferred Qualifications
- Home care experience
- Strong communication, organization, and multitasking skills
Pay
Compensation is hourly, ranging from $22.90 to $34.35.
