LPN - Paid Per Visit, Not Per Hour

About the role

Haven Home Care is expanding its proven home‑care model to Eastern Ohio (Ashtabula, Columbiana, Mahoning, and Trumbull counties). This supervisory LPN position helps establish the agency’s presence in the new territory, grow the client base from zero, and earn supplemental income on a per‑visit basis.

Responsibilities

• Conduct open visits for new clients.
• Perform supervisory visits every 60 days to check on Home Health Aides.
• Complete pre‑employment skills and written tests for aide candidates.
• Prepare accurate and thorough documentation according to agency standards and regulatory requirements.
• Collect and maintain all necessary visit documentation.

Requirements

• At least 18 years old.
• Graduation from an accredited School of Practical Nursing.
• Current Ohio LPN license in good standing.
• Valid driver’s license, reliable transportation, and current auto insurance that covers work‑related driving.
• Current CPR certification.
• Successful completion of a criminal background check, skills test, and written nursing test.
• Residence in Trumbull or Mahoning County preferred.

Qualifications

• One or more years of experience in a home health agency or hospital setting.
• Home health experience preferred.
• Experience with scheduling, documentation, and compliance regulations is a plus.

• Flexible scheduling based on client availability.
• No on‑call hours, but phone calls are required to schedule visits.
• Work builds as the client base grows; initial hours are minimal while the territory is established.
