LPN - Paid Per Visit, Not Per Hour

About the role

Haven Home Care has been a trusted provider of quality home care services since 1998, successfully serving the Akron-Canton area for over 25 years. We are now expanding our proven care model to Eastern Ohio, bringing our established standards and reputation to new communities. This is a unique opportunity to join our Eastern Ohio expansion into Ashtabula, Columbiana, Mahoning, and Trumbull counties. As we launch services in this new territory, we are seeking an LPN to help establish our presence and grow with us.

Responsibilities

• Documentation: Prepare accurate and thorough documentation according to agency standards and regulatory requirements.
• Supervisory and Open Visits: Conduct regular supervisory visits every 60 days to check on Home Health Aides and complete open visits for new clients.
• Pre-Employment Testing: Conduct skills tests and written tests for Home Health Aide candidates.

Requirements

• Must be at least 18 years old
• Graduation from an accredited School of Practical Nursing
• Current Ohio LPN license in good standing
• Valid driver's license, reliable transportation, and current auto insurance that covers driving for work
• Current CPR certification
• Successful completion of criminal background check, skills test, and written nursing test
• Residence in Trumbull or Mahoning County preferred

Qualifications

One or more years in a home health agency or in a hospital setting. Home health experience preferred. Experience with scheduling, documentation, and compliance regulations not required but a plus.

• Must be able to access homes that may not be wheelchair accessible
• Physical activities include walking, sitting, stooping, and standing
• Exposure to various home environments

Pay

• $25 per supervisory visit
• $35 per open visit
• $35 per skills test
• $35 per hour of office time for any/all written tests
• Mileage reimbursement paid at $0.655 per mile
