LPN - Paid Per Visit, Not Per Hour

About the role

Haven Home Care has been a trusted provider of quality home care services since 1998, successfully serving the Akron-Canton area for over 25 years. We are now expanding our proven care model to Eastern Ohio, bringing our established standards and reputation to new communities. This is a unique opportunity to join our Eastern Ohio expansion into Ashtabula, Columbiana, Mahoning, and Trumbull counties. As we launch services in this new territory, we are seeking an LPN to help establish our presence and grow with us.

Responsibilities

• Complete open visits for new clients.
• Conduct supervisory visits to check on Home Health Aides every 60 days.
• Complete pre‑employment testing (skills and written tests) for aide candidates.
• Prepare accurate and thorough documentation according to agency standards and regulatory requirements.

Requirements

• At least 18 years old.
• Graduation from an accredited School of Practical Nursing.
• Current Ohio LPN license in good standing.
• Valid driver’s license, reliable transportation, and current auto insurance that covers driving for work.
• Current CPR certification.
• Successful completion of criminal background check, skills test, and written nursing test.
• Residence in Trumbull or Mahoning County preferred.
• Able to access homes that may not be wheelchair accessible.
• Physical activities include walking, sitting, stooping, and standing.
• Exposure to various home environments.

Qualifications

• One or more years of experience in a home health agency or hospital setting; home health experience preferred.
• Experience with scheduling, documentation, and compliance regulations is a plus but not required.
