About the Role
Provide compassionate, patient-centered hospice care in a home setting.
Responsibilities
- Implement and evaluate care plans
- Manage symptoms and ensure emotional comfort and dignity
- Perform treatments and medication administration within the full scope of nursing practice
- Collaborate with an interdisciplinary team to support patients and families during a significant life stage
- Support safe and comfortable care
- Develop self-care education programs
- Ensure patient safety by adhering to safety protocols
- Maintain competencies and certifications relevant to the role
Requirements
- Graduate from an accredited practical nursing program
- Hold an unencumbered LPN license
- Basic Life Support (BLS) certification
- Preferred one year of nursing experience
- IV therapy skills may be required depending on location
- Valid driver’s license may be necessary
Skills
- Ability to perform testing and interpret data
- Provide age-specific care
- Knowledge of medications, safety hazards, and patient education principles
- Self-scheduling flexibility
Support available for LPNs working towards RN qualification.
