About the Role
Provide patient-centered, comfort-focused care to individuals and families during end-of-life stages in a hospice setting.
Responsibilities
- Implement and evaluate care for stable patients of all ages in a home care setting.
- Support symptom management, emotional comfort, and uphold patient dignity.
- Collaborate with an interdisciplinary team to deliver holistic care.
- Administer treatments and medications accurately, ensuring safety and comfort.
- Develop self-care education programs for patients and residents.
- Maintain safety standards specific to the home environment.
- Provide compassionate, patient-focused care in a hospice environment.
- Participate in team-based care planning and delivery.
- Ensure safety and comfort for patients and families.
- Self-scheduling and flexibility to accommodate rotating shifts, weekends, and holidays.
Requirements
- Graduate from an accredited practical nursing program.
- Unencumbered LPN license with state nursing board or multi-state licensure privileges.
- Basic Life Support (BLS) certification required.
- Preferred: One year of professional nursing experience.
- Knowledge of medications, safety protocols, and patient education principles.
- Ability to perform tests, interpret data, and provide age-specific care.
