LPN Med Care Manager

Role Overview

The Licensed Practical Nurse (LPN) Medication Nurse provides essential nursing support in senior living environments, focusing on safe medication administration, health monitoring, and accurate clinical documentation.

Key Responsibilities

• Administer medications and treatments according to physician orders, following the Six Rights of Medication Administration
• Document all care activities accurately and in real-time within MAR/TAR systems
• Monitor residents for symptoms, administer medications for relief, and report any adverse reactions or refusals promptly
• Conduct Service & Health Updates (SHUs) and transcribe physician orders to support care plan updates
• Observe and report physical, emotional, or cognitive changes in residents
• Maintain medication carts, perform narcotic counts, and ensure compliance with regulatory standards

Qualifications & Skills

• Active LPN license in good standing
• CPR and First Aid certifications
• Strong assessment, documentation, and observational skills
• Experience in senior care or medication administration preferred
