LPN Med Care Manager

Role Overview

The LPN Medication Nurse provides licensed nursing support in a senior living environment, focusing on safe medication administration, health monitoring, and accurate clinical documentation.

Key Responsibilities

• Administer medications and treatments according to physician orders
• Follow the Six Rights of Medication Administration to ensure safety
• Document all care activities accurately in MAR/TAR in real-time
• Monitor residents for symptoms, adverse reactions, and changes in condition
• Report refusals, reactions, or health changes promptly
• Support care plan updates by transcribing physician orders
• Conduct Service & Health Updates (SHUs) and maintain medication carts
• Ensure compliance with regulatory standards and maintain medication security

Qualifications & Skills

• Active LPN license in good standing
• CPR and First Aid certification
• Strong assessment, documentation, and clinical skills
• Experience in senior care or medication administration preferred

The role is vital in preventing medication errors, early detection of health issues, and ensuring resident safety through clinical precision and compliance.
