LPN Med Care Manager

Role Overview The Licensed Practical Nurse (LPN) Medication Nurse provides licensed nursing support in a senior living environment, focusing on safe medication administration, health monitoring, and accurate clinical documentation. Key Responsibilities Administer medications and treatments according to physician orders, following the Six Rights of Medication Administration.Accurately document in MAR/TAR in real-time and transcribe physician orders to support care plan updates.Monitor residents for symptoms, administer medications for relief, and report refusals or adverse reactions promptly.Conduct Service & Health Updates (SHUs), observe and report physical, emotional, or cognitive changes.Maintain medication carts, perform narcotic counts, and ensure compliance with regulatory standards. Qualifications & Skills Active LPN license in good standing.CPR and First Aid certification.Strong assessment, documentation, and clinical skills.Experience in senior care or medication administration is preferred. This role is critical in preventing medication errors, early health change detection, and safeguarding resident safety through clinical precision and adherence to regulations.
