LPN Med Care Manager

Responsibilities

• Administer medications and treatments following physician orders, ensuring accuracy and safety by adhering to the Six Rights of Medication Administration
• Maintain precise clinical documentation in MAR/TAR, monitor residents' symptoms, and report any adverse reactions or refusals promptly
• Conduct Service & Health Updates (SHUs), transcribe physician orders, support care plan updates, and observe physical, emotional, or cognitive changes
• Maintain medication carts, perform narcotic counts, and ensure compliance with regulatory standards
• Play a vital role in preventing medication errors, early detection of health changes, and safeguarding resident well-being

Qualifications & Skills

• Active LPN license in good standing
• CPR and First Aid certification
• Strong assessment, observation, and documentation skills
• Experience in senior care or medication administration is preferred
