About the Role

The Licensed Practical Nurse (LPN) provides comprehensive nursing care to patients, families, and caregivers within a medical practice setting. They collaborate with physicians, registered nurses, and multidisciplinary teams to support patient-centered care and assist with tasks such as case finding, health coaching, and supportive care under the supervision of licensed healthcare providers.

Responsibilities

  • Gather patient measurements and data
  • Record, report, and communicate patient findings
  • Prepare patients for examinations and assist with treatments
  • Support self-care and follow established care plans
  • Maintain equipment, supplies, and a clean, safe environment
  • Participate in quality improvement, continuing education, and team meetings
  • Effectively communicate with the healthcare team and respond to patient requests

Requirements

  • Graduate of an accredited LPN program
  • Current New York State licensure as an LPN
  • Basic Life Support (BLS) certification within six weeks of hire
  • Strong communication, teamwork, and technical skills
