About the Role
The Licensed Practical Nurse (LPN) provides comprehensive nursing care to patients, families, and caregivers within a medical practice setting. They collaborate with physicians, registered nurses, and multidisciplinary teams to support patient-centered care and assist with tasks such as case finding, health coaching, and supportive care under the supervision of licensed healthcare providers.
Responsibilities
- Gather patient measurements and data
- Record, report, and communicate patient findings
- Prepare patients for examinations and assist with treatments
- Support self-care and follow established care plans
- Maintain equipment, supplies, and a clean, safe environment
- Participate in quality improvement, continuing education, and team meetings
- Effectively communicate with the healthcare team and respond to patient requests
Requirements
- Graduate of an accredited LPN program
- Current New York State licensure as an LPN
- Basic Life Support (BLS) certification within six weeks of hire
- Strong communication, teamwork, and technical skills
