About the role

The Clinical Care Coordinator II provides age-specific patient care and assists in managing patient care within a physician office setting under provider supervision.

Responsibilities

• Demonstrates competency in providing age-specific patient care
• Absorbs and implements changes in patient care protocols
• Assists in managing patient care within a physician office setting under provider supervision
• Collaborates with providers to ensure continuity of care
• Ensures compliance with regulatory standards and policies

Requirements

• Basic Life Support/CPR certification within 30 days of hire/transfer
• Graduate of an accredited nursing school or diploma program
• One year of outpatient clinical experience preferred
• Familiarity with electronic practice management and health record systems preferred
• Proficiency in Microsoft Office preferred
• Florida Licensed Practical Nurse (LPN) license required

Qualifications

• High school diploma or equivalent
• Current Florida LPN license

• Strong organizational and time management skills
• Ability to work independently and as part of a team
• Knowledge of medical terminology and procedures
