LPN Clinical Care Coordinator
Primary Responsibilities
- Helps to manage order tracking, IDG preparation, and other tasks associated with the management of patient care.
- Makes sure to monitor pending referrals daily and assigns registered nurse or advanced practice nurse for all start of care visits.
- Maintains productivity and distribution of visits and informs the clinical leader if necessary.
- Schedules most nursing visits to the patient's designated RN Case Manager, Social Worker, to support continuity of care and excellence in patient care management.
- Participates in patient care team meetings to assist with facilitating timely care, via the scheduling process, to patients with symptom crises, transitioning to end of life, or experiencing other immediate care needs.
- Coordinates, schedules, and tracks all visits to ensure compliance with hospice policies and state/federal regulatory requirements.
Required Qualifications
- Current LPN/LVN license in state of practice
- Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation
Preferred Qualifications
- CPR certification or ability to complete within 90 days of hire
Pay and Benefits
Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements).
The hourly pay for this role will range from $27.25 to $40.90 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
