About the role
Monitor and manage client health needs in two supportive housing programs (a 33-unit supportive SRO and a 24-unit supportive SRO).
Responsibilities
- Conduct basic health assessments, including blood pressure check, weight, etc.
- Administer medication.
- Make referrals for medical, psychiatric and substance abuse treatment and rehabilitation programs.
- Act as primary case manager for assigned caseload.
- Develop and run health-related educational groups.
- Participate in treatment team meetings; assist in organizing and participating in goal development and related activities.
- Interact with medical and psychiatric providers; provide staff consultation and related duties as assigned.
- Provide emergency first aid/CPR assistance when needed.
- Reports to Program Director.
Schedule
Full-time 37.5 hours per week Monday-Friday 8:30am-5pm
Qualifications
- LPN license required.
- Experience in geriatric, psychiatric and medical nursing preferred.
- Knowledge of assisted living, home health care and higher levels of care placement preferred.
- Basic computer literacy required.
- CPR training certification or willingness to complete training class in CPR required.
- Vaccination Preferred but not required.
