LPN Case Manager - Robert C. Wood Apartments

About the Role

As the primary medical case manager for assigned clients in a 146-apartment housing program, you will provide essential health services and support to individuals experiencing homelessness.

Responsibilities

• Conduct basic health assessments, including blood pressure checks and weight monitoring.
• Manage client health needs and make appropriate referrals for medical, substance use, and psychiatric treatment.
• Follow up on referrals to ensure continuity of care.
• Develop and facilitate health-related educational groups.
• Participate in treatment team and interdisciplinary team meetings.
• Assist in organizing and participating in goal development and related activities.
• Maintain chart documentation and coordinate with medical, substance use, and psychiatric providers.
• Provide emergency first aid and CPR assistance when needed.

Requirements

• Active NYS LPN license.
• Experience in substance use/addiction, homelessness, psychiatric, and/or medical nursing preferred.
• Basic computer literacy.
• CPR training certification or willingness to complete training.

Schedule

Full time, 37.5 hours per week.
