About the Role
As the primary medical case manager for assigned clients in a 146-apartment housing program, you will provide essential health services and support to individuals experiencing homelessness.
Responsibilities
- Conduct basic health assessments, including blood pressure checks and weight monitoring.
- Manage client health needs and make appropriate referrals for medical, substance use, and psychiatric treatment.
- Follow up on referrals to ensure continuity of care.
- Develop and facilitate health-related educational groups.
- Participate in treatment team and interdisciplinary team meetings.
- Assist in organizing and participating in goal development and related activities.
- Maintain chart documentation and coordinate with medical, substance use, and psychiatric providers.
- Provide emergency first aid and CPR assistance when needed.
Requirements
- Active NYS LPN license.
- Experience in substance use/addiction, homelessness, psychiatric, and/or medical nursing preferred.
- Basic computer literacy.
- CPR training certification or willingness to complete training.
Schedule
Full time, 37.5 hours per week.
