About the role

The role involves providing comprehensive care coordination for patients with complex medical needs, collaborating with healthcare providers, and ensuring patient safety and satisfaction.

Responsibilities

• Coordinate care plans with physicians, nurses, and other healthcare professionals.
• Monitor patient progress and adjust care plans as necessary.
• Ensure compliance with healthcare regulations and standards.
• Communicate effectively with patients and their families to enhance understanding and support.

Requirements

• Bachelor’s degree in Nursing or related field.
• Current licensure as a Licensed Practical Nurse (LPN).
• Experience in case management or related healthcare field preferred.
• Strong interpersonal and communication skills.
