What You'll Do
- Coordinate and implement individualized service and recovery plans.
- Provide community-based case management services and support participants in achieving personal goals.
- Aid participants in accessing resources such as housing, medical care, employment, education, and behavioral health services.
- Advocate on behalf of participants to ensure access to necessary supports and services.
- Transport participants to appointments and community resources when required.
- Collaborate with clinical staff, community providers, and other stakeholders.
- Maintain accurate and timely documentation in accordance with program and regulatory requirements.
- Identify service gaps and communicate recommendations for improving participant services.
- Attend team meetings, supervision, and required trainings.

Qualifications
- Bachelor’s Degree in Human Services or a related field preferred, or equivalent experience.
- Two (2) years of experience supporting individuals in behavioral health, addiction recovery, or intellectual/developmental disability services required.
- One (1) year of case management experience preferred.
Additional Qualifications
- Drivers License with Clean three (3) year driving history
- Ability to work in community settings and transport participants as needed.
- Intermediate computer and documentation skills.
- Must obtain and maintain required background clearances, including Child Abuse Clearance, Criminal Background Check, and FBI Clearance (if applicable).
Physical Requirements
- Ability to drive and travel within the community
- Ability to lift up to 50 pounds
- Ability to sit, stand, bend, and climb stairs as needed
