Case Manager LPN

Responsibilities

• Cook up individualized patient care plans from admission to discharge.
• Conduct ongoing patient assessments and track progress towards care goals.
• Coordinate discharge planning, including arranging post-acute care, home health services, durable medical equipment, and community resources.
• Collaborate with physicians, nurses, social workers, and other departments to maintain seamless patient care.
• Provide education to patients and their families about treatment plans, discharge instructions, and available resources.
• Participate in interdisciplinary rounds to identify and address barriers to discharge.

Qualifications

• Hold a Bachelor’s degree in a clinical healthcare-related field.
• Have at least one year of experience in case management, discharge planning, or nursing management.
• Be a licensed LVN/LPN with at least two years of acute care experience.
• Have basic to intermediate computer skills.
• Have a minimum of five years of acute care experience, preferred.
• Have case management certification within two years of hiring, preferred.
• Have a current BLS certificate, preferred.
• Have knowledge of Milliman Criteria and InterQual Criteria, preferred.
