As a Licensed Practical Nurse (LPN) Case Manager at Stair Crest, you will provide and monitor high-quality clinical care and services to residents, ensuring compliance with regulations and established clinical best practices. Pay

$29.53 - $31.20/hour + BOE

Responsibilities

• Provide and monitor high-quality clinical care and services to residents consistent with regulations and established clinical best practices.
• Support and empower residents to build a vibrant and thriving community.

Qualifications

• Current licensure as a Licensed Practical Nurse (LPN) with the State Board of Nursing in the state(s) of practice.
• Current CPR certification.
• Interest in gerontological nursing, rehabilitative, and restorative clinical practices.
• Demonstrated compatibility with PHS's mission and operating philosophies.
• Ability to read, write, speak, and understand the English language to communicate effectively with all customers.
