Licensed Social Worker - Part Time 24 hrs Variable Shift

About the Role

Provides psychosocial assessments and social medicine services as part of the emergency department and CDU care team, addressing patients' emotional, social, and support needs.

Responsibilities

• Responds to referrals and identifies high-need populations such as those with new diagnoses, terminal illness, homelessness, or complex social issues.
• Conducts psychosocial assessments, develops care plans, and ensures timely documentation.
• Assists patients and families with coping strategies, discharge planning, and connection to community resources.
• Provides consultation on issues like abuse, neglect, advance care planning, and legal decisions.
• Supports coordination of discharge plans and appropriate community placements.
• Offers resource referrals, advocacy, education, and participates in quality improvement and departmental activities.

Requirements

• Master’s degree in Social Work and at least one year of healthcare experience.
• Licensed Clinical Social Worker or Licensed Social Worker in Hawaii.
• Knowledge of medical terminology, care processes, and community resources.
• Strong interpersonal, teamwork, and customer service skills.
• Ability to relate with diverse patients, families, and healthcare professionals.
