Licensed Social Worker - Full-Time Day Shift

About the Role

Provides comprehensive social work services including assessment, care planning, case management, and coordination for patients and families. Acts as a liaison between healthcare providers and the community to integrate services, coordinate care, and strengthen relationships through public relations activities. Initiates social work care plans and offers consultation to staff.

Responsibilities

• Conducts psychosocial assessments, develops and implements social work plans, and treats individuals, couples, families, or groups.
• Focuses on preventing psychosocial dysfunction and improving emotional, mental, and social well-being using theoretical knowledge and functional skills.
• Manages cases, coordinates services, provides resource referral, advocacy, and education.
• Ensures appropriate patient placement and monitors progress.
• Participates in program development, policy implementation, quality improvement, and departmental activities.
• Conducts research, documentation, and clinical record audits to ensure compliance and competency.
• Engages in departmental meetings, staff training, and orientation of new personnel to support team development.

Requirements

• Master's degree in social work.
• Licensed Clinical Social Worker (Hawaii) OR Licensed Social Worker (Hawaii).
• Basic Life Support certification from American Heart Association.

Preferred Qualifications

• Minimum one year of social work experience.
• Experience in healthcare settings with medical diagnoses.
• Strong networking skills with community agencies.
