About the Role
Provides services to patients and their families to address needs incidental to their hospitalization. Offers counseling, support, and assistance in making transition plans. Provides direction for routine non-medical problems of patients and their families. Assists with planning and post-discharge coordination involving outside agencies, nursing staff, and relatives.
Responsibilities
- Collaborates and facilitates patient discharge plans for those requiring post-Hospice or post-inpatient care.
- Assists patient, family, and caregiver in determining the appropriate level of Hospice care. Refers to outpatient resources as needed.
- Initiates a plan of care and appropriate patient teaching. Reassesses the plan of care according to the physical and emotional needs of the patient.
- Develops a Plan of Care (POC) based on patient assessment, including psychosocial factors that may impede the POC, clinical knowledge of human development stages, the impact of terminal illness, mental status, and family dynamics.
- Performs other job-related duties as assigned.
Requirements
- Master’s Degree in social work: Required in CA and HI.
- One to two years of experience in an acute or long-term medical setting: Preferred.
Pay
The estimated base pay for this position is $39.16 to $53.71 per hour. Additional individual compensation may be available through differentials, extra shift incentives, bonuses, etc. Base pay is only a portion of the total rewards package, and a comprehensive benefits program is available for qualifying positions.

- Full-time regular position.
- Day shift.
- 8-hour shifts.
