Licensed Social Worker - Full-Time Day Shift

Role Overview

The social worker provides assessment, care planning, and case coordination services to patients and families, serving as a liaison between healthcare providers and the community.

Responsibilities

• Developing and evaluating social work plans
• Offering psychosocial assessments, diagnosing, and treating individuals, couples, families, and groups to promote social and emotional well-being
• Case management, resource referral, advocacy, and coordination of services to ensure appropriate patient placement and support
• Program development, policy implementation, quality improvement, research, documentation, and participating in departmental activities
• Collaboration with staff and ongoing professional development

Qualifications & Skills

• Master's degree in social work
• Licensed Clinical Social Worker or Licensed Social Worker (Hawaii)
• Basic Life Support certification from the American Heart Association
• Preferred: minimum one year of social work experience
• Preferred: healthcare background with medical diagnoses
• Preferred: strong community agency networking skills
