Licensed Social Worker - Full-Time Day Shift

About the Role

The social worker provides assessment, care planning, and case coordination services to patients and families, serving as a liaison between healthcare providers and the community.

Responsibilities

• Develop and evaluate social work plans
• Provide psychosocial assessments
• Deliver individual, family, or group interventions to address emotional, mental, or behavioral issues
• Case management and resource referral
• Advocacy and coordination of services to ensure appropriate patient placement
• Program development and policy implementation
• Quality improvement and research
• Documentation and participation in departmental activities

Requirements

• Master’s degree in social work
• Licensed Clinical Social Worker or Licensed Social Worker (Hawaii)
• Basic Life Support certification from the American Heart Association

Preferred Qualifications

• Minimum one year of social work experience
• Healthcare experience with medical diagnoses
• Community agency networking skills
