Licensed Social Worker - Full-Time Day Shift

About the Role

Provides social work services including assessment, care planning, case coordination, and consultation to patients, families, and healthcare staff. Serves as a liaison between healthcare services and the community to facilitate resource integration, coordinate care, and strengthen relationships through public relations activities.

Responsibilities

• Conducts psychosocial assessments, develops and implements social work plans, and provides therapy for individuals, couples, families, and groups. Focuses on preventing psychosocial dysfunction and promoting social and emotional well-being.
• Performs case management, resource referral, advocacy, and service coordination to ensure appropriate patient placement and support.
• Participates in program development, policy implementation, quality improvement, and departmental activities, including staff training and record audits.
• Engages in research, documentation, and compliance with regulations, maintaining accurate records.

Requirements

• Master's degree in social work.
• Licensed Clinical Social Worker or Licensed Social Worker in Hawaii.
• Basic Life Support certification from the American Heart Association.

Preferred Qualifications

• At least one year of social work experience.
• Healthcare experience with medical diagnoses.
• Strong networking skills with community agencies.
