Interim Director of Care Coordination
About the role
An Interim Director of Care Coordination is needed to provide leadership and operational oversight for care coordination, case management, utilization review, and social work functions within an acute care setting. Reporting to the Chief Financial Officer, this leader will oversee twenty-five FTEs.
- Mentor the department manager
- Assess departmental operations and recommend opportunities for improved efficiency
- Support scheduling and workflow changes for social work staff
- Advance Medicare-related initiatives
- Ensure effective care coordination across acute care, behavioral health, and women's services
The ideal candidate will have California healthcare experience and previous leadership experience within a similarly sized hospital, with a strong background in case management and care coordination. Must be highly organized, patient-focused, and able to adapt quickly to changing needs. Excellent communication skills will be critical. Must be available to start within 2-3 weeks of acceptance.

- Bachelor's degree required
- Significant healthcare leadership experience in care coordination, case management, utilization review, and/or social work within an acute care hospital setting
- California experience is required

Pay
Compensation Range: $170,000 to $180,000 annually. The final compensation rate will be determined based on experience, education, training, location, internal equity, and budget considerations, in accordance with Fair Market Value evaluation.
