Director Integrated Care Coordination
About the role
The Director of Integrated Care Coordination provides strategic and operational leadership for the Inpatient Care Management Department in alignment with hospital and Jefferson Health enterprise goals. In collaboration with Regional Director of Integrated Care Management and hospital leadership, the Director oversees all departmental operations, growth, and performance.
Responsibilities
- Provides strategic and operational leadership for the care management department, aligning programs and goals with the hospital and Jefferson Health's mission, vision, and strategic plan.
- Develops and oversees departmental structures, policies, and processes to ensure high-quality, efficient, and compliant care coordination, discharge planning, and authorization processes.
- Directs and evaluates departmental operations and performance, including staffing, budgeting, quality initiatives, and integration of clinical best practices across multiple facilities using high reliability organizational principles.
- Collaborates with physicians, hospital leadership, multidisciplinary teams to facilitate effective care progression, discharge planning, and transitions across the continuum of care.
- Serves as a resource for and mentor for managers and team members, fostering professional growth and performance excellence.
- Facilitates implementation of best practices and utilization management processes across the continuum.
- Oversees quality and data-driven performance improvement efforts, analyzing key metrics (e.g. LOS, Avoidable days, discharge barriers) to enhance outcomes.
- Identifies and applies technology solutions to support challenges and develop sustainable process improvements.
- Represents the department and Jefferson Health in internal and external committees, task forces, and professional forums.
- Promotes multidisciplinary collaboration through forums that address regional care delivery strategies and population health initiatives.
- Ensures departmental compliance with regulatory, governmental, and payer requirements.
- Stays current on case management trends, best practices, and industry standards to guide ongoing departmental advancement.
- Encourages continuing education and development for staff to maintain clinical, regulatory, and leadership competencies.
Requirements
- Master’s Degree in healthcare or management or Master’s Degree in social work/nursing/public health or Master’s Degree any with experience in in-patient care management.
- 3 years of progressively responsible hospital management/care management positions related to care management.
- Demonstrated knowledge of inpatient care management management.
- Interpersonal skills necessary to work with physicians and other health care professionals and administrative staff.
- Familiarity with EPIC.
- Ability to use power point, excel and possess strong presentation skills.
- CCM - Certified Case Manager - Commission for Case Manager Certification within 3 Years or ACM - Accredited Case Manager - American Case Management Association Within 3 Years.
Qualifications
