Job Duties
- Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson.
- Leads and manages daily operations of the care management and department across assigned hospitals, ensuring effective, efficient, and compliant service delivery.
- Supervises and develops staff, oversees recruitment, onboarding, scheduling, training, performance evaluations, and corrective actions to maintain a competent and engaged team.
- Collaborates with hospital and enterprise leadership to establish departmental goals, implement policies and procedures, and align with organizational strategies and regulatory requirements.
- Maintains quality, compliance, and performance, auditing documentation, analyzing key metrics, and addressing barriers to patient progression and discharge.
- Serves as a departmental representative and liaison, maintaining communication with hospital leadership, community partners, and external agencies to support patient care and operational effectiveness.
- Oversees payroll accuracy, manages departmental schedules, and ensures compliance with hospital standards and policies.
- Represents the department in hospital meetings, leadership rounding, and projects; serves as acting Director when needed.
- Supports a positive work environment through empowerment, recognition, and professional growth opportunities.
- Coordinates staff orientation, education, and competency assessments; provides continuing education opportunities.
- Ensures new employees meet onboarding goals and performance expectations through chart audits and feedback.
- Conducts documentation audits and analyzes productivity, compliance, and quality metrics to drive improvement.
- Participates in the development, review, and annual update of departmental policies, procedures, and standard work.
- Implements service excellence and patient satisfaction initiatives to maintain high-quality care.
- Ensures adherence to regulatory and clinical requirements for discharge planning and case management.
- Captures and coordinates relationships with hospital departments, post-acute providers, and community agencies to ensure smooth transitions of care.
- Participates in multidisciplinary and escalation meetings to address patient progression barriers.
- Provides regular communication to staff, leadership, and stakeholders regarding departmental updates, priorities, and issues.
- Functions as a resource for complex care cases, offering expertise in discharge planning, psychosocial needs, and patient advocacy.
- Captures and coordinates daily staffing and assignments to ensure adequate hospital coverage and efficient workflow.
Qualifications
- Master’s Degree in social work, nursing, healthcare administration, or related degree, or Bachelor’s Degree with 5 years of acute care management experience and one year as a lead CM or Bachelor’s Degree in Nursing with 2 years experience in acute care management.
- Critical thinking and problem solving skills.
- Preferred qualifications include 2 years in leadership or management roles, experience in word, excel, and powerpoint, and comfort speaking in front of people.
- Knowledge of the conditions of participation for discharging.
- ACM - Accredited Case Manager - American Case Management Association within 3 Years or CCM - Certified Case Manager - Commission for Case Manager Certification within 3 Years.
- Nurses must have an active license in the state of employment; MSWs must have LSW or LCSW in the state of NJ.
